No, it actually is a bug that can be triggered by multiple reasons. The most common is the start of a recording. Then people seems as they walk on top of the map, but they are not - you can still shoot them where they actually are; on the ground. Skywalking on the other hand is not a hack, but in some maps it's possible to skywalk, that is, walk on top of the map, by boosting one another.

Yes. They are exactly where they are shown, just like 20 feet down on the ground. So you can judge where they are by looking at them in the sky. But I recommend waiting until next round before playing to avoid it. It's because the demo recording that starts.
